Anyone who drives a commercial vehicle with a gross vehicle weight rating of 10,001 lbs or more in the state of Alaska is required to have a current DOT physical.  This is a set of one form and one card.  The form is for your physician and you only.  The form does not need to be sent to any state agency or shown to your employer.  The card should be filled out by your physician and must be carried with you at all times when on-duty and shown upon request.
